Interesting to note here his stated reason for exemption from service is that his father was disabled and therefore Frank must provide for his family.
He also stated that his birthplace is Park City, UT instead of Midway, UT

We learn from this document that he was stationed at Camp Kearny in California, he had an eighth grade education, and was not yet married.

This record states that he was inducted in Evanston, WY on 30 Mar 1918. This record also restates his birthplace as Park City, and tells us he did not serve abroad but was honorably discharged 8 Feb 1919.

This record confirms the dates of Frank's enlistment, discharge, birth, and death (although it states his birthplace as Midway, UT) and specifies that he was involved in the Army's Medical Department during his one year service.
